Config I use myself, never been banned for it and I have the same ping range as you, you can turn the reach up a bit if you want but I prefer these settings.

Could be, I have not tried it but I have never had any problems w 16-20 ac and I have never been banned on my main w these settings.
12-16 is just for hitreg.
Hit delay does something different kinda, removing a clientside limit that doesn't allow you to hit anyone if you miss an entity.
Reason you would use 12-16 (obv with nohitdelay too) is because too high cps on certain servers might lead to a bad hitreg. Some people experience bad hitreg on hypixel so they go with a lower cps, which usually improves their hitreg.
