Yep thats just one of the bad things with it, so if you don't know how to use it, it's not a great idea to do so

Kinda but it also depends on angle, aim speed, swing range etc.

On reasonable settings it doesn't really look weird in replay at all.

as for the warning, people have been banned for it (u basically spin around and do 360s when fighting multiple people)
i know someone who got banned for it using these exact settings:
10 aim speed
13.5-15.2 cps
4 swing range
3.1 attack range
60 angle
require mouse down on
